چکیده

Background Facet joints (FJ) arthrosis can be a cause of pain and spinal instability considered to potential target for injection treatment. However, FJ arthropathy is difficult diagnosis confirmation associations with syndrome, effectiveness injections debated. Trigger points (TrPs) are major syndromes, dry needling under ultrasound (US) guidance (DN-US) crucial therapeutic treatment myofascial [1], proven effective method various conditions; restore muscle function motion [2], reduce fascia overload [3], improve movement in spine also FJ. Objectives Aim was evaluate the relevance US as efficacy DN-US treat FJ-associated low back facet joints. Methods We included 26 consecutive patients (15 females, 23-57 years old) symptoms LBP, suspected due uni- bilateral assessed at MRI CT three-dimensional reconstructions. Conditions rheumatic, paresis, advanced injury background were excluded. All received protocol by R. Bubnov [1]: MTrP identified according clinical examination, referred pattern, identification; single fine (28G) steel needle DN applied elicit local twitch response (LTR) and/or `needle grasp`. Specific recommendations given preserve effect after DN-US. Results diagnosed bi- unilateral on levels Th11-S1 all patients, fluid joints, deformation, restriction revealed closely localized TrPs paravertebral (multifidus) muscles followed targeted distinguished different patterns (referred - groin pain, pelvic irradiation leg, thigh, etc.) which correlated localizations. Pain decreased VAS from 6-8 baseline 1-4 immediately 3-4 one week procedure. noted decreasing neuropathic detected higher rates lumbar motility, movements particular segments registered functional US; improvement postural balance Additional sessions capsule did not induce significant LTR alleviating compared had imbalance, multiple MTrPs: multifidus thoracic, levels; sacroiliac joint dysfunction, shoulders impingement, other associated abnormalities; MTrPs inactivated, posture imbalance restored. Conclusion helpful detect arthrosis; has good outcome arthrosis. Local interventions targeting most cases relevant approach; less complicated technically, however, more vs Complex assessment correction needed. References [1]Bubnov RV. [Ultrasound-guided facet block to low back pain: a case report].

BACKGROUND Osteoarthrosis is a common cause of low back pain. The diagnosis is clinical and can be confirmed by imaging studies. Pain treatment and confirmation of diagnosis are made by intra-articular injection of corticosteroid and by local anesthetic use, due to clinical improvement. A direct monitoring of the procedure can be done under fluoroscopy, a classic technique, or else by an ultras...

Ultrasound-guided facet joint injection to treat a spinal cyst.

Lumbar spinal cysts often present with backache and may be accompanied by lower extremity weakness as well as radiation pain if root compression occurs. The treatment regimens include anti-inflammatory drugs, physical therapy, procedures guided by fluoroscopy or computed tomography, and surgery. We report the case of a patient with a symptomatic lumbar spinal cyst at the L4-L5 facet joint. The ...

Ultrasound-Guided Trigger Point Injection for Myofascial Trigger Points in the Subscapularis and Pectoralis Muscles

www.eymj.org We read with great interest the article by Shin, et al. in the May 2014 issue of the Yonsei Medical Journal. The authors described the application of ultrasound-guided (USG) trigger point injection for myofascial trigger points in the subscapularis and pectoralis muscles to post-mastectomy patients. The article was supported by four USG pictures. We, as anesthesiologists, have been...

Facet joints infiltration: a viable alternative treatment to physiotherapy in patients with low back pain due to facet joint arthropathy.

BACKGROUND There is a paucity of information on treatment options and outcome of treatment of patients that have MRI findings of FJA presenting with low back pain in the developing country. This prospective cohort study conducted mainly in the University of Abuja Teaching Hospital Gwagwalada, Abuja Nigeria is to compare the short-term clinical outcome between two groups, one having undergone fa...

Facet Joint Injection for Low Back Pain

Therapeutic injection of facet joints is now widely practised, but British experience has been infrequently reported. We studied the results of injecting facet joints with a corticosteroid preparation in 50 patients suffering from the “facet syndrome”. Our series included a number of exfra-articular injections and these “failed injections” provide a useful control group. Results indicate that o...
